If your child will be trying out for a sport or participating in UIL middle and high school sports, your school will require you to have a physical exam done by their pediatrician. This requirement is fulfilled using the form below for the following public school districts: Plano, Frisco, Prosper, McKinney, Garland. Please check with your school if a different document is required. 

ONLY COMPLETE PAGE 1 of this document. Dr. Singh will complete the second page using information from your Wellness or Consultation visit*.

We will use details from your child's most recent Well Child visit to complete the form. If the date of that visit does not meet your school’s requirements, you may need to schedule a Consult for an updated exam.
