New Patient Appointment Request

Please submit the following form to request an appointment. We will verify your health insurance information and contact you within 1-2 business days to schedule your visit. Be prepared to provide your proof of identification, insurance card, and child's immunization records.

Please wait for our office to contact you for scheduling. We must verify your insurance and create your child's chart before we can schedule an appointment.

We are a Primary Care Clinic with one doctor, and not an Urgent Care center. Please note that New Patient Sickness Visits are offered with extremely limited same-day availability, and are for those who will be establishing long term care with Dr. Anitha Singh.

Insurance

We recommend contacting your health insurance provider to obtain the most updated network participation status, as online lookup tools are not always accurate. It is important for you to understand your insurance benefit details for wellness and sickness visits, as having insurance does not mean that all visits will be covered at 100%.
