CHADIS is our online system that delivers developmental and health questionnaires directly to parents and families. The questionnaire results come directly to your doctor at CareFirst Pediatrics.

This information will help us address any concerns about your child’s health and development. Completing the questionnaire ahead of time also saves you time in the office!

You should complete CHADIS questionnaires:

• The very first time your child visits our office

• Before every well-child visit

• Before all ADHD, asthma, and behavioral health visits

New Users:  Enter our office phone number as the invitation code: 9728678979.

Returning Users:  Log in with your username and password.

• If you have already registered your child on the CHADIS system, please use the username and password you created when you registered.

• If you are unable to complete CHADIS before your visit, please arrive 10 minutes before your appointment time, in order to complete it in the office.
