Parents may complete this form to request immunization records, school notes, health statements, and athletics physicals from the office. Forms needing signatures can be sent to office@carefirstpediatrics.net for the Doctor to complete.

SPORTS PHYSICALS FOR UIL ATHLETICS PARTICIPATION:

- If your child was seen for a Well Check in the past year, please download the UIL Athletics Physical Form, complete the first page, and submit it to our email. Dr. Singh will return the second page to you once the first page has been completed and received. 

- If your child was not seen for a Well Check in the past year or your school needs a more recent physical exam, please request a "Consultation" office visit.

 

The parent must complete the parent/student portion for all forms prior to requesting Doctor's completion. Forms may be emailed to office@carefirstpediatrics.net or dropped off at the office to be picked up the next business day.
