If your child will be trying out for a sport or participating in UIL middle and high school sports, your school will require you to have a physical exam done by their pediatrician. This requirement is fulfilled using the form below for the following public school districts: Plano, Frisco, Prosper, McKinney, Garland. Please check with your school if a different document is required. 

ONLY COMPLETE PAGE 1 of this document. Dr. Singh will complete the second page using information from your Wellness or Consultation visit*.

*For the upcoming 2024-2025 school year, your child will need a physical exam done after May 1st, 2024. If they were seen for a Wellness visit prior to May 1st, 2024, they will need to schedule a Consultation with Dr. Singh to obtain an updated exam. Please check for specific cutoff dates on the form provided by your child's school.
