If you have the time, please complete the newborn paperwork prior to the visit. You may also arrive 10-15 minutes earlier than your appointment to complete these documents in-office.

- Your newborn's discharge paperwork 

- Your baby's immunization record

- Mother's health insurance information (your baby is covered for 31 days)

- Parent's driver's licenses or photo IDs
