NEW PATIENT PAPERWORK

Please download and complete this packet of forms in preparation for your first visit.

Complete one set for each new patient.

MEDICAL HISTORY FORM

Please complete the following form to tell us about family medical history. A medical history form is not meant to replace the doctor’s medical files. However, it a handy reference tool that offers crucial info needed for a period of medical treatment.

PRE-VISIT QUESTIONNAIRE

We use an online developmental and health questionnaire called CHADIS for children age 4 months to 5 years to help us recognize and address any concerns about your child’s health and development.

 You will receive an email to access the questionnaire the week of your appointment, or you may access it here.

VACCINATION POLICY

CareFirst Pediatrics practices the CDC recommended vaccination guidelines. 

If you have chosen not to vaccinate your child, we recommend finding a provider who aligns with your vaccination decisions.
New patients are required to provide immunization records upon scheduling their first check-up.
